IF YOUR CERTIFICATION HAS EXPIRED, DO NOT FILL OUT THIS FORM. CALL OUR OFFICE AT 1-800-832-2726. PAYMENT MUST BE MADE EITHER BY USING GOOGLE CHECKOUT IF YOU REGISTER ONLINE OR BY CREDIT CARD BY CALLING 1-800-832-2726 (ASK FOR BARBARA).
